Feedback Survey Form is a centered hero with a headline, a subline, and a three-step survey card below. The card opens on a radio group where visitors pick an overall satisfaction level. Step two presents a five-star rating row with a label that names the selected score, followed by a dropdown for the features they use most. Step three shows two free-text areas for improvement suggestions and what they like most, plus a second dropdown for referral likelihood. A progress bar across the top of the card tracks position as a step counter and a percentage. Back and Continue buttons move between steps, and the Continue button stays disabled until the required field on that step is answered. On submission the card swaps to a confirmation view with a checkmark icon, a brief thank-you message, two aggregate stats, and buttons to take another survey or return to the dashboard. A privacy note links to a policy at the very bottom of the section.
All copy is straightforward to replace. The three steps are independent, so you can reword each question, swap the dropdown options, or remove the free-text fields without affecting the progress logic. The confirmation stats are static and should be updated to match real figures before going live.
This block works well when you want to collect structured feedback without sending users to a separate survey tool. A fintech or banking product can drop it on a post-onboarding page or after a key milestone like a first transfer or account opening to learn where friction crept in. Because the survey is gated behind a required first answer, drop-off is lower than an open text box, and the star rating step gives you a numeric signal alongside the qualitative comments.
