Submit Request Form is the ticket intake page for Acme support. The main card is a form with a subject field, a product area select, a three level severity radio row (Low, Normal, High) each with a one line hint, a description textarea pre-filled with a worked example, and an attachment row with a file size and a remove action. A footer line shows where replies will go and carries the submit button. A side rail deflects with articles matched to the subject and sets expectations with plan, first response time, and support hours.
The severities, suggested articles, and expectation rows are three typed arrays. The rail pairs deflection with honest response targets, so the form both captures the ticket and tries to answer it before one is filed.
