Scheduled Report Form is the scheduling card for the MRR by plan report, last run Jun 13, 08:00. A two-column row holds Frequency (Weekly selected) and Run time (UTC) (08:00), a second Select picks the day of week (Monday). Recipients render as removable badge chips, avery, priya, and jordan at acme.com, with an inline email input to add more. Three format radios in a ruled list cover PDF, CSV, and Excel with a one-line description each, PDF checked by default. A muted callout at the bottom carries the honest note: scheduled reports send from a no-reply address and recipients cannot reply, so set a real owner in the report.
Recipients and format options are const arrays. The form renders closed Select triggers only, no open Radix portals.
