About this block

Feedback Form Card is the in-app feedback widget: Bug, Idea, and Praise kind chips (pill buttons, Bug selected by default), a Textarea prefilled with a realistic report, the export button stays disabled after the first download finishes, refreshing fixes it, and a screenshot-attached line with a remove link. The card is narrow (max-w-md), centered, and compact so it sits comfortably in a modal or slide-over.

Kinds are a static array. The footer closes with three routing facts, all true and all named: bugs reach the on-call channel, ideas go to roadmap review on Mondays, praise gets read out at the team retro.

When to use

Reach for this block as the feedback entry point inside your app, submitting to your feedback endpoint with the kind, body, and an auto-attached screenshot blob. Route on kind server-side so the footer copy stays accurate.

A natural flow around it on an Application Pro page:

What you can display

One strong use is the floating help-menu feedback tab. Other feedback surfaces:

• NPS prompt. Score 0 to 10 and a one-line follow up, same narrow card shape, shown once per quarter.
• Feature request vote. Idea kind locked, a title Input added above the Textarea.
• Post-deploy survey. Triggered after a successful deploy, kind defaulting to Praise with the option to switch.

Tip: name where feedback goes in the UI itself; users send more when they know a person reads it.
