Form With Channels puts a working contact section on one screen. The left column introduces the team with a heading and a short lede, lists three ways to reach a human as icon rows for sales email, support email, and phone, and closes with the hours someone is actually online. The right column is a bordered card holding the message form itself, with a visible label above every field, a full width pill submit button, and a short privacy note beneath it.
The two columns are independent. Swap the channel rows for your own addresses, change the office hours line, or add and remove form fields without touching the other side. The form is static markup with named, labeled inputs, so wiring it to a server action or your own endpoint is a small change.
Use this block as the heart of a contact page, or near the end of a landing page for a sales led product where a conversation converts better than a signup button. It suits something like a payroll platform, where buyers have specific questions about their own setup and want a named reply from a real person before they commit.
