Map With Contact Info opens with a centered heading and a short location line, then drops into a five-column grid. The left three columns hold an embedded interactive map that fills the full height of the layout. The right two columns are a card with three labeled rows, each with an icon: address with a street and city, a clickable phone number, and a clickable email address. Below those rows, a divider separates a compact inline form with an email field, a short textarea, and a full-width send button. A second divider at the bottom of the card reveals a row of four social icon links for Facebook, Instagram, Twitter, and LinkedIn.
Every text string in the heading, location blurb, address block, phone, and email is plain copy you can swap in place. The mini form and the social row are self-contained sections you can remove independently. On smaller screens the map and card stack vertically, so both work at any width.
Reach for this block when you need to show a physical location alongside a way to send a quick note, without committing a full page to a contact form.
